Reintegration Support Worker (Full-Time)

Our Reintegration Support Service provides person-centred support to people transitioning from prison back into the community. We support people to reconnect with family, access housing, employment, healthcare and other community services that promote successful reintegration and reduce the likelihood of reoffending.

About the Role

As a Reintegration Support Worker, you’ll provide person-centred case management and practical support to people transitioning from custody back into the community.

Reporting to the Reintegration Manager, you’ll work closely with clients and partner agencies to identify individual goals, coordinate supports and advocate for positive outcomes. You’ll also build strong relationships with local organisations to ensure clients can access the services they need to establish safe, stable and independent lives.

This role includes outreach across the Kimberley and may involve travel to support clients in regional and remote communities.

Your responsibilities will include:

• Providing person-centred case management and tailored support.
• Developing individual support plans with clients.
• Assisting clients to access housing, healthcare, education, employment and community services.
• Supporting clients to reconnect with family, culture and community.
• Providing practical support and transport where required.
• Building strong relationships with community organisations and service providers.
• Maintaining accurate client records, case notes and service data.
• Delivering services using culturally safe, trauma-informed and strengths-based approaches.
• Working collaboratively with colleagues to achieve positive outcomes for clients.

Every day is different. You’ll build meaningful relationships, work across the Kimberley and help people overcome barriers so they can successfully reconnect with their families, culture and community.

About You

We’re looking for someone who is compassionate, organised and committed to helping people overcome barriers and achieve positive change.

To be successful in this role, you will have:

• Previous experience in case management, community services or a related field.
• The ability to build respectful relationships with people from diverse cultural backgrounds, particularly Aboriginal people and communities.
• Strong communication and interpersonal skills.
• Excellent organisational and time management skills.
• Experience maintaining accurate records and using Microsoft Office programs.
• The ability to work independently while contributing to a collaborative team.
• A commitment to culturally safe, trauma-informed and person-centred practice.

The successful applicant will also be required to obtain:

• A Department of Justice Criminal History Screening.
• A National Police Clearance.
• A Working with Children Check.
• A current Western Australian “C” Class Driver’s Licence.

Experience working within justice, corrections or reintegration services is highly regarded, however we also encourage applications from people with transferable experience within the community services sector.
